How many days before a period does PMS start?

Usually three to seven days out, and worst in the final forty-eight hours. But the number that matters is hers, not the average.

The usual window

Premenstrual symptoms generally appear in the last week of the luteal phase — commonly three to seven days before bleeding starts — and resolve within a day or two of the period beginning.

Severity is rarely flat across that window. For most people it builds, and the final forty-eight hours before onset are the steepest part, because that is when the hormone withdrawal is sharpest.

Why the average is not much use

Both the length of the window and its intensity vary substantially between people, and somewhat between cycles for the same person. "Three to seven days" is a population statistic; the person you actually live with has a narrower and more consistent pattern than that range implies.

You find it the same way you find any pattern: by writing down when periods start, for a few cycles, and looking at what happened in the days before. Three logged cycles is usually enough to be useful; one is not enough to be anything.

What to do with the number once you have it

Work backwards from it. If the hard stretch reliably begins five days out, then the useful moments are: the shopping around day seven, the difficult conversation postponed past day five, and a deliberately lower-friction couple of days at the end.
